You may preregister with our office by filling out our secure online Patient Registration Form. After you have completed the form, please make sure to press the Submit button at the bottom to automatically send us your information. On your first visit to our office, we will have your completed form available for your signature. The security and privacy of your personal data is one of our primary concerns and we have taken every precaution to protect it.
Your initial appointment will consist of a consultation to discuss your diagnosis and treatment options. Occasionally, consultation and surgery can take place on the same day. Patients with a complex medical history or treatment plan will require an evaluation and a second appointment to provide treatment on a another day.
Please assist us by providing the following information at the time of your consultation:
- Your consultation/surgery referral slip and any x-rays if applicable.
- A list of medications you are presently taking.
- If you have dental insurance, please bring your insurance card as well as your photo ID. This will save time and allow us to help you process any claims.
IMPORTANT: All patients under 18 years of age must be accompanied by a parent or legal guardian to all consultation and surgery appointments.
A preoperative consultation and physical examination is mandatory for patients undergoing IV anesthesia for surgery. All patients undergoing IV anesthesia are not to have anything to eat or drink eight hours prior to surgery and must also have a responsible adult that stays on the premises throughout the procedure and drives them home.
Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e., diabetes, high blood pressure, artificial heart valves, artificial joints, rheumatic fever, etc.) or if you are currently taking any medication (i.e., heart medications, aspirin, anticoagulant therapy, etc.).
If your dentist or physician has current x-rays, you may bring a copy of them with you to your appointment or request that they forward them to our office. If additional films are necessary, they can be taken at our facility.
If you are in pain or an emergency situation, every attempt will be made to see you as quickly as possible.
We try our best to stay on schedule to minimize wait time. Because Dr. Austin provides many types of dental services, various circumstances may lengthen the time originally allocated for a procedure. Emergency cases can also arise and cause delays. In these instances, we appreciate your patience and understanding.
